Admission and Discharge

As Theriak Medication Management (TMM) can be fully integrated with the EPJ system, all relevant patient related data can be sourced from there. TMM compiles all documentation necessary as a patient is discharged and transfers them to the electronic patient record. This contains information, such as: Medication overview, treatment card,release medication and prescription card. If appropriate this can be facilited through the use of a wristband, issued during administration, containing the patients medical and personal information on a barcode.

TMM allows for a fully automated discharge process, saving the discharging doctor valuable time.  User group reviews have shown a reduction in discharge times of up to 40%.

In addition, at discharge the patient can be provided with either discharge protocols or drug information to help their understanding of the next steps in their treatment and hopefully reduce the risk of readmission. This is often also combined with the provision of discharge information to GPs or other primary care practitioners.
